A modern tragedy.

A hound, whipped cream, and unbridled gluttony.

After a hectic few weeks and a very long stretch without a day off, I definitely needed this reminder. Here’s to progress every day ✨

Meet Ava! ❤️

This sweet girl is a middle aged mini Australian shepherd who was presented to me this past week for further evaluation of presumed tracheal collapse. She had noted intermittent difficulty breathing over the past 3-4 years, worst with activity or exercise. Importantly, she really did not cough at all. A palatoplasty was performed earlier this year, which improved her signs by about 10-20% per the owners.

The first video is Ava walking about 10 feet down the hallway…. As you can hear, she has significant airway noise and stridor leading to concern for a fixed upper airway obstruction. She was able to calm herself after this, with some improvement but no resolution of her signs.
As Ava remained stable (ish), we first performed video fluoroscopy where you can see marked extrathoracic tracheal narrowing. Due to this as well as the obstructive pattern and pretty severe impact on Ava’s life, a tracheal stent was then elected. And you can hear the difference it made for her post operatively!

Ava is a perfect case example of when a tracheal stent is truly warranted and beneficial to the patient. It will NOT improve chronic coughing from tracheal collapse, and likely will make it worse due to the foreign object implanted. But, for cases of severe airway obstruction a stent can be life saving.

While there are risks/potential complications of the stent, and progression of her disease later in life, this procedure will greatly improve Ava’s quality of life. Her owners were so gracious to allow her videos and diagnostics to be used for teaching purposes, so give Ava some love!
